A full page copy of your passports ID page. It must be valid for at least the next 6 months when you apply for the visa, with at least two blank pages remaining. The passport should be in acceptable condition. If not there is a chance your application would be rejected.
The photo should be (45mm x 35mm) taken against a white background (the background must be white!). If you cannot get a photo yourself then we can use digital copy.

Kindly ensure all the fields of the visa application form are completely filled before submitting. If a question does not apply, please put ‘N/A’. You must fill in the form with black ink and using BLOCK CAPITALS.
The following nationals DO NOT require a visa for Azerbaijan for stays of up to 90 days: Azerbaijan, Belarus, Georgia, Kazakhstan, Kyrgyzstan, Moldova, Russia, Tajikistan, Ukraine & Uzbekistan

Azerbaijan transit visas are required by all except those as noted above and those continuing their journey to a third country within 72 hours & not leaving the airport transit area, provided holding confirmed onward tickets.
A reciprocal health agreement for medical treatment exists with the United Kingdom in Azerbaijan. In order to obtain treatment some proof of United Kingdom residence such as a UK passport will be required. Further details are available from www.dh.gov.uk. Theare are no compulsory immunisations in place, however, it is recommended that travellers making 3 or more visits per year, stays of more than 3 months in a rural area, high-risk occupational groups & backpackers staying more than 1 month are up to date with Diphtheria, Hepatitis A, Hepatitis B, Malaria, TB, Tetanus, Tick-borne encephalitis & Typhoid.
Malaria is a risk from Jun-Oct in the southern border areas and the Khackmas region of the north.
